Not reading the question carefully: Leaving Cert exam questions often have multiple parts. Missing a part means losing easy marks. Read the entire question before starting, and answer all parts.
Leaving answers in unsimplified form: Examiners expect final answers in simplest form. Leaving sqrt(12) instead of 2*sqrt(3) may lose marks for not completing the calculation.
Not showing working: Even if your final answer is wrong, correct working earns partial marks. Examiners cannot award marks for invisible reasoning. Write out each step evidently.
